|  | | Steve Allen is a graduate of the Univeristy of Southern California 1977 with a MS in Physical Therapy. Internships included attending the Kaiser Vallejo Rehab Program with Margaret Knott and Greg Johnson for intense instruction in Proprioceptive Neuromuscular Facilitation. Steve is the founder of Evergreen Physical Therapy in Spokane and currently serves as Clinic Director at Therapeutic Associates, Inc. in Liberty Lake, Washington. Steve received his Board Certification in Orthopedic Physical Therapy in 1986 and is a 1996 graduate of NAIOMT. Steve received his Fellow in the American Academy of Orthopaedic Manipulative Therapy in 1998. Steve presently serves on the Board of Directors for NAIOMT. Steve is available for part time residency training and also serves as Clinic Instructor for Eastern Washington University and the University of Southern California. |

|  | | Kathy graduated from the University of North Carolina, Chapel Hill in 1978 with a BS in physical therapy and in 1980 with a MA in athletic training and sports medicine and certification as an athletic trainer. She is currently completing her DSc in orthopedic manual physical therapy. Kathy began studying manual therapy at Michigan State University in 1981 and became involved with the founders of NAIOMT in 1989. She became an orthopedic clinical specialist in 2000 and a fellow of the American Academy in 2002. She served as the director of physical therapy at Southwest Michigan Sports Medicine Clinic from 1980-1994. She has taught at Andrews University since 1989 and became the orthopedic coordinator in 1994. In 2003 she became the director of the postprofessional degree program at Andrews University and in 2004 she received the Daniel A. Augsburger Excellence in Teaching Award. She has served NAIOMT as a lab assistant with Erl Pettman from 1991-2007 and as a faculty in training since 2007. She became a NAIOMT faculty member in 2009. Kathy has had multiple presentations at professional meetings and currently sees patients at a chronic pain clinic. |

|  | | Alexa Dobbs received her BS in Physical Therapy from the University of Washington in 1974. She earned her OCS from the American Board of PT Specialties in 1993, her COMT from NAIOMT in 1994, and her Fellowship with the American Academy of Orthopedic Manual Physical Therapy in 1999. She is also a member of the board of examiners for the American Academy of Orthopedic Manual Physical Therapy and for the North American Institute of Orthopaedic Manual Therapy. She has been involved in clinical education program supervision of physical therapy students and in teaching the general public since 1980 and has been a member of the faculty of the North American Institute of Orthopaedic Manual Therapy since 1998 and served as treasurer of NAIOMT. The Orthopaedic Clinics of North America published three of her articles in 1999 and 2000. She has conducted seminars on the physical therapy approach to management of head, neck, and facial pain to dentists, physicians, and other healthcare professionals. She has managed her own private outpatient clinic in Seattle since 1983 and now works at Advanced Manual Therapy & Sports Rehabilitation in North Seattle where she is available for patient care, consultation, teaching and mentorship. Alexa loves to tap dance, hike, and ski and serves as a volunteer ski instructor for Outdoors For All. |

|  | | Laura received her Masters of Physical Therapy degree from Eastern Washington University in Spokane, Washington in 2000. She was then recognized as an Orthopeadic Clinical Specialist by the American Board of Physical Specialists in Spring of 2005 and certified as COMT by NAIOMT December 2005. She was accepted as a CFI for NAIOMT in 2006 and received her FAAOMPT in December 2007. Laura has recently moved to Brussels, Belgium to start up her clinic as she continues to be active and supportive with NAIOMT. She will also continue to return back to the US not only for NAIOMT courses but also to provide clinical work and mentorship for Olympic Physical Therapy in Seattle as well as mentoring to outside therapists. She encourages therapists to seek certifications within any of their current manual therapy programs as well as offering guidance to therapists outside her company. She specializes with manual therapy for pediatric and adult sport and spine injuries. |

|  | | Kent Keyser received his BS in Physical Therapy from Cal State Northridge/UCLA in 1978, certification in Athletic Training in 1983, and a Masters Degree in Human Anatomy from the University of Oregon in 1987. He became OCS certified in 1995 and received his COMT in 1996. He is a graduate of the NAIOMT Residency Program, receiving his Fellow with the American Academy of Manual Physical Therapy in 1998. He is also on faculty and has taught extensively with the Institute of Physical Art and is a Founding Fellow of CFMT. |  | | Michael Lucido is a 1985 graduate of the University of Texas Southwestern Medical Center and possesses over 22 years of orthopedic physical therapy experience with clinical interest and specialization in spine dysfunction. Michael is presently employed by Select Medical Inc. and is presently the program director of the spine mentorship/specialist program in the Dallas/Ft. Worth market and serves on Selects national spine faculty. He has been involved with entry level and post-professional physical therapy education since 1992. He has served has adjunct faculty and guest lecturer for both physical therapy programs at the University of Texas Southwestern Medical Center and Texas Woman’s University. Michael is presently on clinical faculty for the sports medicine fellowship program for primary care physicians at Charleton Methodist Medical Center in Dallas and recently published a chapter in the Experts Guide to Sports Medicine, concerning principles of orthopedic physical therapy and rehabilitation. Michael was recognized as an Orthopedic Clinical Specialist by the American Board of Physical Therapy Specialists in 1995. He was certified by the North American Institute of Orthopedic Manual Therapy (NAIOMT) as an orthopedic manual/manipulative therapist in May 2003. In March 2005 Michael graduated from NAIOMT’s clinical fellowship program and was accepted as a “Fellow” in the American Academy of Orthopedic Manual Physical Therapist and is a faculty-instructor for NAIOMT. |  | | Laurie graduated from McMaster University in Hamilton Ontario in 1982 and received her Advanced Diploma of Manual and Manipulative Physiotherapy from the Canadian Physiotherapy Association with honors in 1988. She has developed and taught continuing education courses in Canada since 1987 and joined the NAIOMT Faculty in 1992. More recently she has taught courses in Europe and Australia. Since 1992 she has been a National Examiner for physiotherapists specializing in orthopaedics in Canada and was a Chief Examiner for Canada from 1999 to 2005. Laurie is also a NAIOMT Examiner. |  | | Jim has 30 years of clinical and teaching experience throughout North America, Britain and Norway. His first teacher of manual therapy was James Cyriax and he continued his learning in Norway and Canada, earning his certificate in Manipulative Therapy in 1983. He was chair of the Canadian Orthopedic Divisions Education and Specialization Committees for 12 years and a past Examiner and Instructor with the Canadian Orthopedic Division. Jim is a co-founder and a Senior Faculty Member and Senior Examiner with the North American Institute of Orthopedic Manual Therapy. He is a past faculty member of the University of Alberta and current adjunct faculty at the Texas Womens University, Andrews University, MI, the Massachusetts Institute of Health Sciences, MA and the University of Maryland at Baltimore, MD. He also visits and lectures at the University of Pittsburgh, PA. He has presented at National and International Conferences on Manual Therapy and has published in numerous journals. His video series Manual Therapy and textbook Orthopedic Differential Diagnosis in Physical Therapy are sold throughout North America. |

|  | | Bryant Miller earned his MPT from Emory University in 1993, a DScPT from Andrews University in 2007, holds a manual physical therapy certification through NAIOMT, and has been awarded fellowship status in the AAOMPT. As co-owner of Bodies in Balance Physical Therapy PLLC, Bryant specializes in chronic spinal dysfunction, and has extensive experience in gait analysis as well as orthotics design and modification. His interests include clinical reasoning in differential diagnosis and the interrelationships of the lower extremity and spine function in walking and running gait. Bryant’s background includes advanced training in myofascial techniques and movement re-education as a Hellerwork practitioner. He has been active in the defense of spinal manipulation at the state legislative level and chronicled those experiences in an editorial published in The Journal of Manual & Manipulative Therapy, Vol. 15 No. 3 (2007), E64–E72.Bryant previously competed at a national level in running and triathlons and currently enjoys rock climbing, hiking and photography. |

In addition to being a Senior Faculty with NAIOMT, he has been a certified manual therapy instructor for the Olaf Evenjth system since 1990. His chapter on Upper Cervical Instability was published in Modern Manual Therapy, 2nd Ed 1992 and he has presented papers at IFOMT Proceedings in 1984, 1988 and 1992. Erl Pettman developed the integrated quadrant system of evaluation in 1980, a three-month manual therapy course in Western Canada in 1982 and founded the NAIOMT in 1984. He is currently practicing in Abbotsford, BC, Canada, and is researching early intervention work conditioning. |

|  | | Ann Porter Hoke received her physiotherapy diploma from St Thomas Hospital, London in 1971, a BSc PT in 1972 from the U.B.C, Vancouver and a transitional doctoral degree in PT from Pacific University, OR in 2008. She is a Fellow of both the Canadian and American Manual and Manipulation Academies and is a Board Certified Orthopaedic Specialist. Her awards include: Mercedes Weiss Award, OPTA, Oregon (1996) and the John McM. Mennell Service Award, AAOMPT in 2006.
She trained and worked with J. Cyriax MD in London, England from1967-72; with advanced manual therapists in Vancouver, BC 1972-82, and has been in clinical practice in Oregon since 1983, currently at New Heights Integrative Therapy in SE Portland. She has served on numerous OMPT committees including the Standards Committee for IFOMT, Standards and Examination Committees for the AAOMPT, and has been involved in the development of important publications that include the Description of Advanced Specialist Practice in OMPT (1998 & 2008) and IFOMT Standards 2008.
Her publications include: Causes of Lumbar Instability and Biomechanics of the Cervical Spine, both published in Ortho PT Clinics of NA, Acceptance Speech for the Mennell Award in JMMT 2007, and she has developed NAIOMT course manuals over 16 years.
She is the director of the NAIOMT Fellowship Program, a senior faculty and examiner, with 33 years of teaching and 38 years of manual therapy clinical experience. She teaches all levels of NAIOMT core classes in mobilization/manipulation, specialty classes on spinal instability and subacute MVA, and provides advanced clinical training and clinical reasoning instruction, primarily in Oregon and Washington. |

He received his Bachelor of Medical Rehabilitation (Physical Therapy) from the University of Manitoba, Winnipeg in 1981 and completed the Advanced Orthopaedic Manual Therapy Program at the University of British Columbia, Vancouver two years later. Brett Windsor graduated as a physiotherapist from Curtin University in Perth, Western Australia, in 1994. After working briefly as a physiotherapist in outpatient sports and orthopedics in Australia, Brett spent time working in the Borders region of Scotland before moving to the United States in1995, where he worked in acute care through Indiana and Michigan before beginning practice in outpatient orthopedics in Fort Wayne, Indiana.
Brett began studying with the North American Institute of Orthopedic Manual Therapy (NAIOMT) in 1996. He became APTA board certified as an orthopedic clinical specialist (OCS) in 2000, completed the NAIOMT Manual Therapy Clinical Fellowship in 2003, which included time studying in Montreal, Quebec, and became a fellow of the American Academy of Orthopedic Manual Therapy (FAAOMPT) in 2004. He is now a faculty member for the NAIOMT organization. In addition to teaching courses, Brett mentors clinical fellowship students, and maintains an active clinic practice at various locations in the Pacific Northwest. In 2006, Brett published a clinical case study on peripheral manipulation techniques (HVLAT) to the elbow in the Journal of Manual and Manipulative Therapy.
Brett has extensive teaching experience, both in the United States, and overseas. He is currently an Assistant Professor of Physical Therapy at Pacific University in Forest Grove Oregon and an adjunct Assistant Professor of Physical Therapy at Saint Francis University in Loretto, Pennsylvania and has presented programs at both state and national meetings. He is the chief instructor for manual therapy courses taught to Italian physiotherapists at the Fondazione Turati Rehabilitation Institute, in Gavinana, Tuscany, Italy.
Brett is a member of the Washington State Physical Therapy Licensing Board and recently completed a Master’s Degree in Public Administration from Portland State University in Portland, Oregon. He is a volunteer physical therapist for La Center High School and resides in La Center, Washington with his wife Susan.
